چکیده

We report a case of supra-annular mitral valve replacement for a patient with prosthetic ring endocarditis that developed late after undergoing undersized mitral annuloplasty (UMAP). A 58year-old man had undergone left ventriculoplasty and UMAP with a 26-mm semi-rigid full ring for dilated cardiomyopathy and functional mitral regurgitation. The mitral valve had progressively stenosed and he developed prosthetic ring endocarditis 5 years after the operation. In the surgery, there were brownish fresh vegetations around the anterior mitral leaflet and the prosthetic ring. The mitral annulus was very small, even after removal of the infected ring prosthesis and debridement of the infected tissues. We implanted a 22-mm mechanical prosthesis in the supra-annular position using everting mattress sutures to prevent the valve leaflets from getting stuck by the thickened subannular tissue. Herein, we discuss the surgical procedures for a shrunken mitral annulus.

The edge-to-edge technique for mitral valve repair

The edge-to-edge technique was introduced in the surgical armamentarium of mitral valve repair in 1991 and has progressively been used to restore mitral competence in the setting of degenerative, post-endocarditis and functional mitral regurgitation. Appropriate indications and awareness of the important technical aspects of the procedure are prerequisites for a good outcome. The free edges of ...

Treatment of prosthetic valve endocarditis complicated by destruction of the aortic annulus.

PURPOSE It has been reported that surgical treatment for prosthetic valve endocarditis complicated by destruction of the aortic annulus is associated with high mortality and morbidity. The aim of this study was to evaluate the efficacy of our surgical strategy for this situation. METHODS Between October 2003 and April 2009, eight patients (mean age 68.6 years) with prosthetic valve endocardit...

Effects of undersized mitral annuloplasty on regional transmural left ventricular wall strains and wall thickening mechanisms.

BACKGROUND Undersized mitral annuloplasty, widely used for ischemic and functional mitral regurgitation (MR), has been proposed as an "annular solution to a ventricular problem." Beyond relief of MR, it is thought to improve global left ventricular (LV) shape, hence potentially reducing myocardial stress and promoting beneficial reverse LV remodeling. We previously observed that undersized annu...

Surgical management of functional tricuspid regurgitation with a new remodeling annuloplasty ring.

BACKGROUND Moderate-to-severe functional tricuspid regurgitation (TR) should be corrected in patients undergoing surgery for left-sided valvular diseases, to improve long-term outcomes. Several techniques of surgical repair (suture annuloplasty or prosthetic annuloplasty) to correct this condition have been described. Multiple clinical studies have shown the superiority of prosthetic remodeling...

Valvular Heart Disease Comprehensive Annular and Subvalvular Repair of Chronic Ischemic Mitral Regurgitation Improves Long-Term Results With the Least Ventricular Remodeling

Background—Undersized ring annuloplasty for ischemic mitral regurgitation (MR) is associated with variable results and 30% MR recurrence. We tested whether subvalvular repair by severing second-order mitral chordae can improve annuloplasty by reducing papillary muscle tethering. Methods and Results—Posterolateral myocardial infarction known to produce chronic remodeling and MR was created in 28...
